Digital Passports for people with learning difficulties

Do you have learning difficulties? or do you know someone who does? University Hospital of Morecambe Bay (UHMBT) has launched digital passports to support patients who have learning difficulties. A hospital passport provides important information about patients care and communications needs, including personal details, the type of medication they are taking, and any pre-existing health conditions.
