Get free 1-to-1 coaching from our Health and Wellbeing Coach to support you to make changes to your health and wellbeing.

A health coach could provide support to improve your wellbeing and improve your health outcomes for the future.

Health Coaching is here to help you find a way to live and feel well whatever twists and turns life brings. When you arrive at our team, you’ll be assessed by one of our trained coaches. They’ll talk to you about the help you feel you need and the goals you want to achieve, and help you move forward to where you want to be.

How does Coaching work?

You will receive one-to-one support for up to 13 weeks.  The support will be tailored to your needs and will focus on what matters to you!

Your Health Coach will help you to build your skills, knowledge, and confidence so that you become an active participant in your own health management by helping you to set realistic goals. You will be provided with relevant resources and connected to appropriate services within your local community before your coaching sessions end.  

Your Coaching sessions can be conducted in person, over the phone or online via WhatsApp or similar. You will arrange the times and days to meet with your Coach so that you know when to expect support. Once your sessions have ended, your Health Coach will contact you within 6 weeks for a brief check in.

  • Working out what your skills and strengths are
  • Developing your existing knowledge on your health and wellbeing
  • Building confidence
  • Increasing motivation
  • Dealing with life pressures
  • Connecting you to services and activities in your community
  • Making plans for the future

Your conversations with your coach are confidential.

Our Health and Wellbeing Coach

Tanya is trained to provide inclusive, friendly, flexible support. Tanya has many years of supporting people through her work with Lancashire Women and other Mental Health charities.

As the Health and Wellbeing Coach for LDCVS, Tanya is looking to support people with respiratory conditions or those who are at risk of developing them. Tanya is passionate about supporting people to improve both their physical and mental health.

Tanya is aware that there are various factors that could impact on a person’s health and is motivated to help individuals break down barriers, build resilience and ultimately see the best in themselves to move towards a happier and healthier future.

Getting started

To start your coaching journey, please do get in contact. We accept self-referrals, or you can be referred by a health professional.

We have put together a helpful eligibility checklist. If you feel that you are eligible for the support, all you have to do is complete our online form.

And if for any reason you can’t do this online, give us a call on 01524 555900 and we will be happy to help you. Our team will then be in touch.
